What it is

Ashwagandha (Withania somnifera) is an adaptogenic herb used for millennia in Ayurvedic medicine and now well-studied for its effects on the stress response. An adaptogen helps the body modulate its reaction to stress — buffering the peaks and troughs rather than stimulating or sedating directly.

Its primary studied action is on the HPA (hypothalamic–pituitary–adrenal) axis and the regulation of cortisol, the body's main stress hormone. Its active constituents are a group of compounds called withanolides.

Why standardization matters

Ashwagandha's effects depend on its withanolide content — the active compounds responsible for its activity. Raw or poorly processed root powder varies widely in potency, so a quality product is standardized to a defined withanolide percentage and prepared as a concentrated root extract. Well-characterized standardized extracts are what the clinical studies have used. The dose that matters is therefore the standardized extract dose, not simply milligrams of powder.

Taken with food, it is gentle on the stomach. If it is being used primarily for daytime stress resilience rather than sleep, a morning or twice-daily schedule may be chosen instead — your physician sets the timing to your goal.

Safety note — thyroid, immune and sedative cautions

Ashwagandha is biologically active and warrants the following specific cautions:

  • It can raise thyroid hormone levels. Use caution if you have hyperthyroidism, and monitor closely if you take thyroid medication, as the effects can be additive.
  • As an immune modulator, it warrants physician guidance in autoimmune conditions (for example thyroid autoimmunity, lupus or alopecia areata), where immune stimulation is a theoretical concern.
  • It may add to the effect of sedatives, sleep medication, benzodiazepines and alcohol — separate or avoid the combination unless cleared by your physician.
  • It can mildly lower blood sugar and blood pressure; monitor if you take medication for either.
  • Avoid during pregnancy. It belongs to the nightshade family, so anyone with a known nightshade sensitivity should review the product first. Choose a quality, standardized extract and discontinue if any unusual symptoms arise.
