What it is

Nattokinase is an enzyme extracted and purified from natto — a traditional Japanese food made from soybeans fermented with the bacterium Bacillus subtilis. Its defining property is fibrinolytic activity: the ability to act on fibrin, the fibrous protein your body uses to form blood clots.

In healthy biology, clot formation and clot breakdown are held in constant balance. With most supplements the number on the label is a weight — so many milligrams. Nattokinase is different: what matters is not how much enzyme is present, but how active it is. Why evening, and why empty stomach

Nattokinase is an enzyme, and like all enzymes it can be drawn into the work of digesting a meal if taken with food. Taking it on an empty stomach — at least one hour before eating or two hours after — keeps its activity available for circulatory support rather than spent on digestion. Evening or bedtime dosing is commonly used because the body's own fibrinolytic activity naturally dips overnight and into the early-morning hours. Exact timing is set by your physician based on your individual profile and current medications.

Safety note — drug–nutrient interaction

Nattokinase has genuine fibrinolytic and mild blood-thinning activity. Because of this, the following points are essential, not optional:

  • If you take anticoagulant or antiplatelet medication — for example warfarin, a DOAC such as apixaban or rivaroxaban, aspirin or clopidogrel — do not start nattokinase without direct physician supervision. The combined effect can meaningfully increase bleeding risk.
  • Discontinue nattokinase at least one to two weeks before any planned surgery or dental procedure, and inform your surgeon or dentist.
  • Use caution if you have a bleeding disorder, a history of hemorrhagic (bleeding) stroke, or active stomach ulcers.
  • Other blood-thinning supplements — fish oil, ginkgo, garlic, high-dose vitamin E — add to its effect. Review your full supplement list with your physician.
  • Because nattokinase is derived from soybeans, anyone with a soy allergy should review the product before starting.
